Employees shall use this form to report all work related injuries, illnesses, health challenges or “near miss” events (which could have caused an injury or illness) – no matter how minor and no matter whether they occurred at work or outside of work – any even that could in any way affect the contractual obligations or health and safety at work or home. It can also be used for vehicle related incidents. This helps us to identify and correct hazards before they cause serious injuries. This form shall be completed by employees as soon as possible and the submission will be investigate by a Tour Manager / Supervisor for further action.
