Incident Report

Employees shall use this form to report all work related injuries, illnesses, or “near miss” events (which could have caused an injury or illness) – no matter how minor. It can also be used for vehicle related incidents. This helps us to identify and correct hazards before they cause serious injuries. This form shall be completed by employees as soon as possible and the submission will be investigate by a Tour Manager / Supervisor for further action.

Make sure all your statements are 100% truthful. Any misrepresentation or untruths on this form will lead to disciplinary action and/or dismissal.
